Replacement iPhone 3G USB Power Adapters Shipping

On September 19, Apple announced a voluntary recall and replacement program for the USB power adapter that ships with the iPhone 3G. For those who ordered a new one online, they have started to ship. Replacements will also be available in Apple retail stores starting October 10.

According to the Mac|Life on Monday, the new adapters can be identified by a green dot on the plug side.

Apple requires that the old adapter be shipped back within ten business days, or the customer's credit card will be charged. For those who visit an Apple retail store, they'll need to bring their iPhone 3G and the old power adapter.

Apple said that the old adapter prongs, in some cases, could break off within the electrical socket leading to possible shock.

Does anyone know if you need to bring in the cord too, or just the little square adapter?

I believe it is just the square adapter from what I read on the apple site.
